Concussion Return To Play Protocol

Security checks for vulnerabilities and agentic risk

Overview

This skill is a clearly disclosed draft concussion return-to-sport planning aid for qualified clinicians, with explicit limits that it must not clear an athlete to play.

Install only if this will be used by or under a qualified concussion-management clinician. Treat outputs as draft documentation, not medical clearance, and ensure all Stage 5/6 clearance decisions and signatures are completed outside the agent by the supervising HCP according to the applicable governing-body rules.

Vulnerability Patterns
  • Skill Instruction HijackingAlters the agent's session goals or safety constraints when the skill loads
  • Agent Memory PoisoningWrites attacker-controlled rules into memory that affect later sessions
  • Remote Payload Retrieval and ExecutionFetches external code whose behavior can change after review
  • Embedded Malicious CodeShips malicious scripts inside the skill and executes them locally
  • Unauthorized Access and Privilege EscalationObtains permissions beyond the task's legitimate needs
Vulnerability Patterns
  • MCP Tool PoisoningHidden Instructions, Unicode Deception, Parameter Description Injection
  • Prompt InjectionInstruction Override, Hidden Instructions, Exfiltration Commands
  • Data ExfiltrationExternal Transmission, Env Variable Harvesting, File System Enumeration
  • Privilege EscalationExcessive Permissions, Sudo/Root Execution, Credential Access
  • Supply ChainUnpinned Dependencies, External Script Fetching, Obfuscated Code

Finding

The skill repeatedly states it never clears an athlete to play, but the workflow and output template still scaffold Stage 6 return-to-sport and include a return-to-sport confirmation field. In a medical context, that contradiction can normalize or facilitate premature clearance by producing documentation that appears to endorse progression through full-contact practice and game return.

Finding

Although the skill says clearance is reserved for an in-person supervising clinician, it instructs generation of full-contact and return-to-sport clearance workflow sections that closely resemble an operational clearance form. Because concussion management is safety-critical, this structure could be reused as de facto authorization or pressure clinicians toward progression despite the stated boundary.
