Incident Postmortem Framework

Security checks for vulnerabilities and agentic risk

Overview

This skill is a simple postmortem-writing template, with the main caution that users may paste sensitive incident details into it.

Install only in an environment approved for your incident data. Before use, redact secrets, tokens, customer personal data, private Slack content, and regulated information unless your organization allows that data to be processed by the agent.

Vulnerability Patterns
  • Skill Instruction HijackingAlters the agent's session goals or safety constraints when the skill loads
  • Agent Memory PoisoningWrites attacker-controlled rules into memory that affect later sessions
  • Remote Payload Retrieval and ExecutionFetches external code whose behavior can change after review
  • Embedded Malicious CodeShips malicious scripts inside the skill and executes them locally
  • Unauthorized Access and Privilege EscalationObtains permissions beyond the task's legitimate needs
Vulnerability Patterns
  • Data ExfiltrationExternal Transmission, Env Variable Harvesting, File System Enumeration
  • Prompt InjectionInstruction Override, Hidden Instructions, Exfiltration Commands
  • Privilege EscalationExcessive Permissions, Sudo/Root Execution, Credential Access
  • Supply ChainUnpinned Dependencies, External Script Fetching, Obfuscated Code
  • Excessive AgencyUnrestricted Tool Access, Autonomous Decision Making, Scope Creep

Finding

The skill explicitly encourages users to paste raw Slack threads, verbal notes, and incident details, which commonly contain sensitive internal information, credentials, customer data, or regulated operational details. Because the skill provides no warning, redaction guidance, or data-handling constraints, it increases the likelihood of inadvertent disclosure to the agent or downstream systems.
